Specific Need
Our purpose is Caring for Our Neighbors Every Way, Every Day. Our team works together to provide clients with the highest level of care and compassion. Providing multiple services in one building available during business hours allows for an increased level of care coordination which in turn improves outcomes. Funding provides community support needs and wellness in Pottawatomie, Wabaunsee and Surrounding Counties.
Currently, we offer medical, limited mental health, food assistance, and social assistance services. Our medical, and limited mental Health services are available to uninsured individuals whose household income is at or below 200% of the Federal Poverty level. There are no charges for eligible patients/clients for in-house services.
Our Food Pantry serves those in need in in the county. We also provide Commodity Supplement Food Pantry program for Seniors, and the Emergency Food Assistance Program or Commodities to whose household income is at or below 130% of Federal Poverty level. Our Social Service assistance includes assistance with rent, utilities, helping displaced community members when there is a fire, disaster, or life-altering event, as well as finding resources that would help a person start or keep working.
Without funding CCM could not reach as many low-income people in need. Preventative and acute healthcare practices, chronic disease management, preventative, rent and housing assistance, and food distribution would not be possible to those in need. To continue these services, we need your support and generous monetary gifts.

We do this by assisting residents from our surrounding area to inspire individual wellness; to encourage healthy family patterns, ensure access to affordable health care, and enlist others to work together for the common good. All in-house services are free to eligible clients and patients.

Community Care Ministries is a non-emergent primary care safety net clinic. Our non-emergent primary care services include but are not limited to:

Food Assistance
CCM provides the following distribution options to assist with food shortages individuals and families may experience. Clients must complete CCM’s Application/Intake process for eligibility. There are no costs or fees for our food assistance distributions.

CCM Food Pantry: our in-house food pantry option can be accessed once per month and is open to individuals or families. Must complete Application/Eligibility process for eligibility.

Senior Box Commodities: This option is for those 60 years of age and older. Income, Application/Intake process requirements are required.

EFAP USDA Commodities: Families and individuals can access this option every other month. Income, Application/Intake process requirements are required.
